HRSA Posts Grant Notice

HRSA within HHS on September 29, 2020, posted the Grant Notice (HRSA-21-027) to enable grant applicants to apply for funding under the Rural Health Care Services Outreach Program (Outreach Program).

The Outreach program is a community-based grant program promoting rural healthcare services by enhancing healthcare delivery to rural underserved populations in local community or regions.

The Outreach Program will:

  • Expand the delivery of healthcare services to include new and enhanced services exclusively in rural communities
  • Deliver healthcare services through a strong consortium, in which every consortium member organization is actively involved and engaged in the planning and delivery of services
  • Utilize community engagement and evidence-based or innovative informed models in the delivery of healthcare services
  • Improve population health by demonstrating health outcomes and sustainability

 

In 2019, the HHS Rural Health Task Force developed the Healthy Rural Hometown Initiative (HRHI). The HRHI addresses the underlying factors that are driving growing rural health disparities related to the five leading causes of avoidable death.

The Federal Office of Rural Health Policy (FORHP), the Bureau Office within HRSA, strongly encourages applicants seeking funding for the Outreach Program to pursue the HRHI track with the applicant’s community needs.

FORHP intends to fund approximately 15 HRHI applicants with estimated total program funding to be $12,750,000 with proposal applications due December 1, 2020.  HRSA will select the top ranked applications from the HRHI Track and the Regular Outreach Track when making funding decisions.  

Eligible applicants must be domestic public or private non-profits or for-profit entities, The applicant organization may be located in a rural or urban area but must have demonstrated experience serving or have the capacity to serve rural underserved populations.
