HRSA https://www.hrsa.gov posted Funding Opportunity titled “Delta States Rural Development Network Grant Program” (HRSA-20-087) on August 27, 2019 with an estimated total program funding of $12,000,000. The closing date for applications is December 6, 2019.
The Delta Program supports developing integrated healthcare networks to address unmet local healthcare needs and prevalent health disparities in rural Delta communities. The Delta program supports organizations in eight Delta States including Alabama, Arkansas, Illinois, Kentucky, Louisiana, Mississippi, Missouri, and Tennessee to expand access, improve the quality of essential healthcare services, and strengthen the rural healthcare system as a whole in the region.
HRSA requests applicants to propose multi-county/multi-parish projects that address the delivery for preventive or clinical health services for individuals with or at risk of developing chronic health diseases. Since individuals with chronic disease are at risk for depression and other mental health conditions, applicants are encouraged to address mental health and substance use disorder, including opioid disorder.
Applicants should focus on no more than two areas such as diabetes, cardiovascular disease, obesity, acute ischemic stroke, or HIV/AIDS. Programs should also include activities related to wellness screening measures.
Recipients are encouraged to explore reimbursement mechanisms for chronic care management and align care management strategies with reimbursement mechanisms to improve health outcomes and reduce costs.
