Geisinger https://www.geisinger.org, an integrated health services organization, and Merck https://www.merck.com, a global biopharmaceutical company have just launched two innovative web-based workflow solutions.
The “Family Caregiver Application” provides two way communications and coordination of care for patients and their caregivers. The “Family Caregiver Application” features a calendar, a patient managed medication scheduler, information on healthcare team members, and enables users to write notes and have dialogue during the healthcare encounter.
This App was developed and piloted in one of the most complex therapeutic areas such as oncology and is designed to be translated to other therapeutic areas to improve coordination between family caregiving activities.
The other workflow solution the “MedTrue Application” integrates different medication data sources to assist with medication reconciliation and adherence resulting in a patient and healthcare team verified medication list.
The “MedTrue Application” features the ability to fill medication data, has a patient web interface for patient reported medication verification, information on changes and adherence accessible in the home or at a healthcare facility, and point-of-care access to the patient’s validated medication list and adherence.
Both Apps developed can be embedded directly within the EMR and in the healthcare provider’s workflow for optimal use of clinical and patient-reported data during patient appointments. The Apps will use an open standards-based platform so data can be utilized from disparate EMRs.
In addition to being tested within Geisinger prior to launch, real-world experience studies are being conducted to gather additional evidence on each solution’s effectiveness. Plans are for Geisinger and Merck to begin the next phase actively exploring the expanded use and commercialization of these Apps.
