The Health Resources and Services Administration (HRSA) www.hrsa.gov within HHS, posted a funding opportunity grant opportunity on June 14, 2017 with applications due July 17, 2017. The grant opportunity notice is titled “Delta Region Community Health Systems Development” (HRSA-17-117). The grant notice estimates total program funds to be $2,000,000.
This grant opportunity notice seeks applications to improve healthcare delivery in the Delta Region by improving hospital or clinic financial operations. The goal is to provide intensive technical assistance to Critical Access Hospitals, small rural hospitals, Rural Health Clinics, and other select number of identified communities in the Delta Region.
The goal is to promote an evidence-based culture to improve and upgrade health outcomes, enhance coordination of care, to increase access and the use of telehealth, to address gaps in clinical service delivery, strengthen the local healthcare system to improve population health, and address how affordable broadband can be made available to the region.
In addition, the Delta Region needs social services pertaining to patients such as housing, child care, energy assistance, access to healthy food, support for the elderly, job training, workforce recruitment and retention resources, and to upgrade emergency medical services.
The awardee will work with a number of selected communities in the Delta Region to make in-depth health system enhancements by providing technical assistance over multiple years. The awardee will collect data to identify all of the high need communities in the Delta Region, and for HRSA.
Eligible applicants can include state, county, city governments plus other government entities, public and state controlled institutions of higher education, nonprofits, plus tribes and tribal organizations, and faith-based and community-based organizations.
