The application extends Qualcomm Life’s web-based HealthyCircles Care Coordination Platform to provide a software-as-a-service (SaaS) solution that includes secure communications, record sharing, and management tools for care team coordination.
This solution is important because while mobile health is becoming increasingly more prevalent, the majority of apps and devices on the market are mainly health and fitness trackers that usually only provide consumer generated health data.
These fitness trackers aren’t for people that need trackers the most such as chronic care patients. As healthcare moves from the hospital to the home and the industry transitions away from episodic care, patients and healthcare professionals are in need of a continuous care model.
