Transitions of Care Service Launched

Hielix a professional services provider for the healthcare industry and Rural Health Innovations (RHI), a for-profit subsidiary of the National Rural Health Resource Center together have launched a new Transition of Care service tailored to small and rural healthcare providers to help them meet the requirements in Stage 2 of Meaningful Use.

Meaningful Use Stage 2 requires both hospitals and eligible professionals who transition their patients to another setting of care or to a new provider of care to submit a summary of care record for each transition of care or referral electronically.

The Transition of Care service will help provide healthcare providers with the tools that they can immediately use to enhance patient outcomes. The service will use the Direct exchange project to provide secure communications for the exchange of key patient records. These may include the discharge summary, care plan, referrals, lab results, or other documents.

In addition, a clear development plan will be created for implementing the Transitions of Care solution based on national standards, to each stakeholder group. The service will also help authorized providers obtain a Direct email address and provide training on its use including where Direct integrates with transitions of care.
