Expanding Hospital@Home Care

NTT DATA https://us.nttdata.com headquartered in Tokyo, has a solution to help U.S. healthcare systems develop new care models that expand care into a patient’s home.

Hospital @ Home programs serve patients with conditions that meet the criteria for hospitalization yet can be safely treated at home. The Hospital @ Home care model is a key solution that addresses many of the current struggles of health systems and patients. This is due in part to the successful implementation and adoption of telehealth, a Hospital @ Home perquisite, which skyrocketed during the pandemic.

The American Hospital Association reports that Hospital @ Home care makes economic sense, generates positive experiences. It also allows for a more flexible working environment for healthcare providers.

While the idea of house calls is not new, transitioning care out of the hospital is complex and requires strategic planning. Healthcare systems and providers must determine whether patients have adequate and persistent broadband connectivity, can collect and manage the medical data that travels from one endpoint to another, and be able to implement well considered operational practices for the use of that data.

NTT DATA’s Hospital @ Home solution enables at home care technologies to work seamlessly with the hospital. The technologies are grounded with software that bridges administrative and clinical data.

Three Key functions are incorporated to treat patients at home:

  • The Command Center interconnects patients and clinicians, provides intelligent management for remote devices, provides rollup of care activities to a single encounter, helps manage communications and AI to predict future needs.
  • The acute rapid response capability collects and transmits data for accurate real time care through remote means, manages information, supports audio and video capabilities, provides for management of connectivity issues and privacy and security issues, establishes partnerships for tech installation and support, and provides for patient/clinician engagement.
  • The use of clinical innovations provides ways to advance remote care delivery capabilities through robotics and automation, augmented and virtual reality tools, remote imaging options, smart beds, and other advanced in home monitoring technologies, analytics, and AI

According to Richard Swafford PhD., Healthcare Industry Consulting Director, NTT DATA Services, “Moving care into the home requires thinking through a myriad of details and mastering new workflows in order to deliver the quality care at home that patients require. The good news is that it is not a far-future innovation. Hospital @ Home is achievable with today’s technology for health systems that are willing or needing, to take the next steps.”
