Expanding Care at Home

The Advanced Care at Home Coalition http://www.achcoalition.com, is working to advance care to allow more patients to receive acute-level care and recovery services in their homes.

The Coalition advocates for the continuation of current hospital-level care at home during and beyond COVID-19. The goal is to look beyond the public health emergency and look into creating an advanced care at home delivery model at the Center for Medicare & Medicaid Innovation (CMMI). The Coalition seeks to expand access to this care model by addressing regulatory and legislative barriers.

The plan is to empower more patients to receive high quality acute and restorative care in their homes. The Coalition also strives to share best practices, provide leadership, and help additional health systems across the country participate in this new model of care delivery.

Founding members of the Coalition includes Mayo Clinic, Kaiser Permanente Adventist Health, ChristianaCare, Geisinger Health, Integris, Johns Hopkins Medicine, Michigan Medicine (University of Michigan), Novant Health, ProMedica, Sharp Rees-Stealy Medical Group, UNC Health, and Unity Point Health.

As an example of a successful program providing advanced care at home, Mayo Clinic physicians advancing care at home operates via Mayo Clinic’s Platform which is operated by Mayo’s Center for Digital Health. At home services includes skilled nursing, medications and infusions, as well as laboratory, imaging and rehabilitation services from nurses, paramedics, and support team members.
