HHS Releases Grant Forecast

HHS https://www.hhs.gov on October 17 released the Forecast for the Funding Notice Empowering Older Adults with Disabilities through Chronic Disease Self-Management Education Programs Financed by the Prevention and Public Health Fund” with total program funding estimated to be $6,390,383.

The HHS Administration on Aging https://www.hhs.gov/aging/index.html within the “Administration for Community Living (https://acl.gov) posted the forecast with possible funds to be included in FY 2019 funds. The funding will enable three year grants to be awarded to about 10 entities to bring programs so that older adults and adults with disabilities to better manage their chronic conditions.

ACL intends to provide these ten grants via two options:

  • Sustainable Systems Grants—Approximately 6 grants for $500,000 to $900,000 to focus on developing integrated and sustainable systems to deliver chronic disease self-management education and self-management support programs
  • Capacity Building Grants— Approximately 4 grants for $50,000 to $150,000 to introduce and deliver evidence-based chronic disease self-management education and self-management support programs to underserved areas and/or populations

 

Eligible applicants can include state, city, county, special district, and Native American tribal governments, independent school districts, institutions of higher education, public and state controlled institutions of higher education, plus nonprofits.
