Partners HealthCare https://www.partners.org has deployed a new telehealth initiative called “Partners HealthCare On Demand” with Teladoc Health https://www.teledochealth.com . Partners HealthCare is now offering a new solution for urgent care which consists of video visits, provided by Partners clinicians for minor illnesses and injuries.
Partners HealthCare On Demand will be exclusively offered to commercial members of the system’s Neighborhood Health Plan (NHP) an NCQA accredited not-for-profit health plan providing coverage to commercial and MassHealth members. NHP will become AllWays Health Partners on January 1, 2019 and will connect patients to providers through secure video visits through the web or through a mobile device.
The Partners telehealth initiative is being managed by Partners Connected Health in collaboration with Partners Community Physicians organization and NHP. Partners Connected Health also has programs addressing medication adherence, care coordination, chronic disease management prevention, and wellness.
