Partners to Deliver Remote Care

Life 365 Inc., www.life365inc.com, a digital health platform provider is joining with American Medical Response (AMR) https://www.amr.net, a provider of ground and air medical transportation, to deliver remote care management services for under engaged healthcare populations in some cases requiring complex care.

Life365’s digital platform will manage connectivity and technology  so that AMR’s mobile integrated healthcare program will be able to obtain advice from an inter-professional workforce from over 35,000 clinicians and support staff covering 47 states and the District of Columbia.

Life365 will provide health monitoring tools to be used in the home. Connected medical devices, two-way video, patient education resources, health surveys, medication reminders, plus digital therapeutics, and other tools to boost patient engagement will also be used.

AMR’s operational infrastructure will enable paramedics, nurses, and other care staff to care for patients by addressing each individual’s needs and identifying gaps in care. AMR’s care teams will help transport patients to appropriate levels of care when and where needed whether the patient needs to go to the ER, to an urgent care center, or to an appointment with a primary care physician.
