CDC named HealthInsight http://healthinsight.org as one of ten recipients nationwide that have signed a five year cooperative agreement for $6.8 million to expand coverage and participation in CDC’s National Diabetes Prevention Program. HealthInsight is partnering with a number of groups to increase the infrastructure to reach in-person and online individuals with diabetes.
HealthInsight is partnering with a broad array of groups to increase infrastructure and reach people in person and online CDC recognized organizations in 23 in rural, underserved counties in New Mexico, Oregon, and Utah. The emphasis is on reaching priority populations with the specific focus on engaging Medicare beneficiaries, American Indian and Latino individuals.
It is important to specifically promote uptake and spread of diabetes self-management education opportunities in disadvantaged communities. HealthInsight will provide technical assistance, training, and financial support to new program providers.
Value-based pilots will be implemented with health plans and employers. The plan is to develop the business case for making the National Diabetes Prevention Program lifestyle change series a covered benefit for their employees or members.
Also, regional, virtual learning sessions will be held for stakeholders and partners which will include training on how to become a Medicare Diabetes Prevention Program supplier, how to bill for services, and provide the opportunity to share challenges and best practices.
