AMC Health www.amchealth.com has been selected as the telemonitoring provider for HealthPartners Institute http://HealthPartners.Institute to continue their high blood pressure study. Previous phases of the Hyperlink study have shown positive results in the control of blood pressure with the use of home telemonitoring.
“Over 85 million adults have high blood pressure”, reports Karen Margolis MD with HealthPartners. “Only about half of people with high blood pressure have their condition under control. Our study will show whether telehealth improves hypertension care for patients who are enrolled directly from a real-world primary care setting.”
Recently HealthPartners was awarded $6 million from the Patient-Centered Outcomes Research Institute (PCORI) www.pcori.org which will allow for the expansion of the study to include a larger sampling of patients.
In the second phase of research, AMC Health and HealthPartners will work together to monitor patients’ blood pressure from home along with a direct link to pharmacists or nurse practitioners. This is in addition to regular doctor visits to determine if telemonitoring can improve a patient’s health and decrease blood pressure more than traditional clinic-based care.
AMC Health will provide patients with in-home devices to measure their blood pressures which will automatically send vitals data through AMC’s CareConsole® clinical platform into the HealthPartners Epic EMR. As part of the care team, pharmacists will review, analyze, and act upon home blood pressure readings to manage patients’ hypertension.
