RI Starts New Data Service

The Rhode Island Quality Institute (RIQI) www.riqi.org  and Integra Community Care Network http://intergacare.org have launched their “Care Management Alerts and Dashboards.” This is an innovative new service enabling healthcare providers to proactively coordinate care and support 140,000 individuals throughout the state.

This is greatly needed as Rhode Island is a small state, and as a result, there is plenty of patient mobility between healthcare systems with patients often going to different hospitals and practices.

“This new service help providers stay connected with others by delivering care to their patients in a timely way that prevents patients from falling through the cracks when seen in different care settings in the state,” said Laura Adams, President & CEO for RIQI.

The new system is able to provide near real-time data about patients admitted to and/or discharged from acute care hospitals and emergency departments across the state. The Integra Care Management Dashboard, helps the clinical staff at Care New England, Rhode Island Primary Care Physicians Corporation, South County Health, and care management staff at Integra have quick and easy access to data, improves care coordination, allows for time-sensitive follow-up, and provides more education and support for their patients.

Each time an Integra patient is admitted to or discharged from a participating hospital, a skilled nursing facility, or from an emergency department, Integra’s care teams receive the information alerting them about their patient’s status. In addition, care teams can view more detailed information on current as well as past admissions and discharges for their patients.

It is also possible for Integra’s care teams to watch trends over time for specific patients and track outcomes. This is particularly valuable for following their patients with chronic conditions or follow an unusually high number of hospital visits,

“Having alerts and access to records from other facilities enhances our ability to manage care for our patients,” said Ruth Scott, Director of Care Management for the Integra Community Care Network. “It can also help with medication management as well as lab and radiology results particularly when there are multiple providers and locations.”

“It is common sense that doctors are able to provide the best care when they have a patient’s full medical history in front of them”, said Senator Sheldon Whitehouse who helped launce RIQI while serving as Rhode Islands’ Attorney General.
