MFH Announces Special Projects Grants

The Missouri Foundation for Health (MFH) www.mffh.org Special Projects Grants provide support for community-based improvement projects in MFH communities. The Special Projects program supports one time efforts to identify health needs in communities.

Examples for funded projects includes:

  • Telehealth programs for home care and hospice
  • Population health programs
  • Care coordination partnerships and models
  • Workforce pipeline training tracks for high school students
  • Adult vaccine initiatives
  • Therapy programs for children who have abused substances and children with disabilities

 

501(c) (3) organizations and state and local governments are eligible to apply. Ideas of any size and scope may be submitted for consideration. Budgets need to align with the scope of the project

Funding for $8,000,000 will be available. The Letter of Intent is due February 5, 2016 with the full application due April 18, 2016.
