Tandigm Health www.tandigmhealth.com works with Philadelphia Primary Care Physicians (PCP) to help reduce duplicate services, improve patient satisfaction, and help prevent health crises that can drive up costs through emergency room visits.
The company is now partnering with the mobile health company called Touch Care www.touchcare.com which enables more than 350 Philadelphia area PCPs to connect to their patients from their mobile device or tablet.
The partnership enables TouchCare’s HIPAA-compliant mobile telemedicine app available from the App Store http://itunes.apple.com and Google Play http://play.google.com, to deliver healthcare to people when needed.
The two companies are providing an additional way for doctors to securely communicate with their patients using mobile devices. The video option supplements in office appointments, provides test results, medication refills, helps with chronic illness management and much more.
The first phase of the partnership will focus on select high-risk patients who already engage telephonically with Tandigm Health’s Care Team. This provides patients with remote real-time face-to-face interaction.
The first phase will allow Tandigm Health medical directors to use TouchCare to work with participating PCPs to assist in care coordination. As the program continues, TouchCare will be rolled out to additional Tandigm Health patients for both provider-to-patient and provider-to-provider communications.
