The American College of Clinical Pharmacy (ACCP) has issued a Request for Applications (RFA) www.accpri.org/CMMRFA to improve performance in primary care practices by implementing a Comprehensive Medication Management (CMM) grant program.
ACCP expects to fund $2.5 million for two research proposals and is looking at the impact of CMM by qualified clinical pharmacists embedded in primary care practices as integral members of the healthcare team. ACCP seeks information and knowledge on how CMM programs can be delivered efficiently in primary care practices with pharmacists working as a team to improve CMM.
Several issues need to be addressed:
- What patients and population within primary care practices would benefit most from CMM?
- What is the impact of CMM on quality of care and cost?
- What can the clinical pharmacist contribute to helping primary care practices?
- How does the clinical pharmacist contribute to the net revenue generated by the practice and the return on investment?
The goal is find out if CMM can be delivered effectively, and efficiently in real-world practices and how healthcare professionals and office staff within busy practices will be able to work together to improve outcomes
Online inquiries will be accepted at www.accpri.org/CMMRFA. The Letter of Intent is due May 5, 2015, informational conference calls will take place on May 5, 2015 at 12:00 pm ET, and on May 11, 2015 at 4:00pm ET.
Online inquires and Letters of Intent will be accepted until June 1, 2015. Notification to submit a full proposal will be sent out on June 15, 2015. Proposals will be due on or before August 1, 2015 and applicants will be notified of a final decision by September 15, 2015.
