The New York State Department of Health (DOH) on December 5th announced the selection of regional contractors for the “Population Health Improvement Program (PHIP). The PHIP was the result of a proposal in Governor Cuomo’s 2014 State of the State address that reflects the recommendations of the Public Health and Health Planning Council. Eleven regional contractors will work with community stakeholders to develop innovative health promotion strategies.
The goal of the PHIP is to promote better care, better population health, and promote lower healthcare costs. The program is a regionally-based, data-driven effort to bring together stakeholders that are impacted by health and healthcare issues within eleven regions of the state.
The following PHIP contractors selected for awards are:

The Finger Lakes Health Systems Agency will serve as the PHIP contractor in the Finger Lakes region and will provide technical assistance to DOH that will help support the other contractors.
The contractors will collect, analyze, and use outcome and claims data from all appropriate available sources including public health, regional health information organizations, or other qualified entities to identify local needs and to measure local health system performance.
They will also advance prevention priorities in coordination with local health departments and hospital planning activities included in county health department community health assessments, community health improvement plans, and hospital community service plans.
The contractors will develop training and coaching programs to enable physicians, nurses, hospital administrators, and other healthcare professionals to effectively practice transformation, analyze problems in care delivery, and successfully design and successfully implement solutions.
