Vermont Issues Several RFPs

The State of Vermont Health Access in their Blueprint for Health Division is seeking qualified bidders to respond to an RFP that is looking for “Full Time Quality Improvement Practice Facilitators for Primary Care and Practices Providing Substance Abuse Treatment”. The RFP was issued initially on March 25, 2014 but revised on April 4 2014.

The Blueprint for Health’s objectives are to achieve primary care practice transformation, the implementation of community health teams to coordinate care across health services, and to expand the treatment for substance abuse and co-occurring mental health disorders.

To support the program, the Blueprint has developed the team initiative called “Expansion and Quality Improvement Program” or EQUIP, which consists of trained individuals known as facilitators to use their skills to improve care. EQUIP facilitators work with multi-disciplinary teams in primary care practices to continuously implementing and managing quality improvement.

The RFP (03410-134-14) is specifically soliciting applications for facilitators to serve primary care practices in a geographic area, plus a specialized practice facilitator to support practices and programs across the state, and provide technical assistance to other facilitators.

The second RFP (03410-128-14) originally issued in February but amended April 1, 2014 involves information technology. The Department of Vermont Health Access (DVHA) is soliciting competitive bids from qualified vendors for fixed price proposals for a “Care Management Solution”, involving software design, developing, implementing, and maintaining the project for the Vermont Agency of Human Services.

DVHA through this procurement is also seeking to acquire supplemental care management operational and clinical support services for DVHA’s Vermont Chronic Care Initiative. The care management solution will need to integrate with Vermont’s Medicaid Management Information System.
