CHCF Accepting Applications

Community Health Center and clinics want to build their capacity to care for the twenty million Americans who will be newly insured under the ACA. To help them, the California HealthCare Foundation (CHCF) in partnership with the Kresege Foundation is accepting applications for the “Partnering for Impact Initiative” from U.S based companies to collaborate on the current issues that community health centers face.

Grants for up to $1.5 million will be awarded to companies with technology or service-based solutions to enable community health centers and clinics to stretch their resources, increase patient access to care, and/or lower the cost of providing care. The foundations will identify and invest in companies that already serve health centers as well as those that are motivated to bring their solutions to underserved markets.

In addition to the funding, selected companies will be paired with community health centers and clinics ready to implement new solutions and connect with mentors experienced in community health center management, technology, and public payment issues.

To be eligible, companies must be able to demonstrate how their product or service can maintain or improve the patient experience while enhancing health center efficiency by at least 20 percent. They must also provide access to highly needed health services for a hundred thousand patients annually and/or reduce costs by $25 million annually.

There is no deadline in submitting an application. Submissions are reviewed on a rolling basis and if the review team determines that the technology or service offered is a good fit, than a more complete detailed application form will be required.
