Request for Proposal Issued

The Minnesota Department of Health (MDH) issued an RFP for the “Minnesota Accountable Health Model e-Health Grant Program”. The grant funding is to advance the Minnesota Accountable Health Model and to prepare for potential participation in accountable communities for health.

The Minnesota Accountable Health Model resulted from a State Innovation Model (SIM) grant awarded by the Center for Medicare & Medicaid Innovation that went to MDH in 2013. The vision of the state accountable health model is to enable every patient to receive coordinated, patient-centered primary care.

To achieve shared cost and coordinated care, the Minnesota Accountable Health Model includes key investments so that accountable care models will be successful in using e-health as one area for investment. The grant funding will help Community Collaboratives develop a plan to meet e-health requirements and to implement and expand e-health capabilities.

Community Collaboratives are eligible to apply for the funding but individual organizations are not eligible. A community collaborative must have at least two or more organizations participating in or planning to participate in an accountable care organization.

Funding to be available in 2014 will be used for development purposes and is available for up to $785,000 per grant with 3-5 grants to be awarded. Up to $1,000,000 per grant with 3-5 grants will also be awarded to implement the program. In 2015, it is estimated that there will be up to $2 million available for implementation grants only. A 20 percent match is required.

This RFPs is one of the three e-health RFPs to be released using SIM funding. The other RFPs include the “Minnesota e-Health Roadmaps to Advance the Minnesota Accountable Health Model”, and the Minnesota Technical Assistance and Education: Privacy, Security and Consent Management”.
