PatientPoint Tracker Introduced

PatientPoint, Inc. has released their PatientPoint® Tracker to enable hospitals and other providers to track patients across the care continuum by identifying gaps in care. PatientPoint serves more than 61,000 physicians across all programs and more than 570 hospitals throughout the U.S

By using customized dashboards, hospitals can follow the patient from the time an appointment is scheduled through the check-in process and visits to one or more departments. This can be followed with post care tracking such as follow-up appointments.

Because PatientPoint Tracker integrates with a hospital’s Admissions-Discharge-Transfer (ADT) system, there is minimal data entry required from the staff. The system is also integrated with the PatientPoint Care Coordination Platform to track patients across the care continuum.

Miami Children’s Hospital is currently using the system to track patients from the time they enter the patient access area through their visits to service units. The dashboard provides clear visibility to current patient wait times so that managers can use the software tool to adjust staff levels as necessary. “We are now better able to integrate our patient flow and gather data points on the experience across the care continuum,” reports Ed Martinez, CIO, at Miami Children’s Hospital.
