AFrame Delivers Patient Telemonitoring

The Virginia Hospital & Healthcare Association (VHHA) through its shared services affiliate Virginia Hospital Shared Services Corporation (VHHA Services) has selected AFrame Digital Inc. to deliver their patient telemonitoring solution to use in Virginia’s hospitals and health systems.

The technology’s broad range of capabilities will help healthcare providers engage high-acuity/high-risk patients and their families, improve care coordination, and optimize the allocation of our members’ clinical resources,” stated VHHA Services Vice President David Jenkins.

The MobileCare Monitor is able to integrate telehealth, activity and patient reported survey and medication compliance data on a single FDA cleared platform to help care teams quickly assess each patient’s health and well-being.

Beneath an intuitive web interface lies a data analytic engine that continuously and non-intrusively evaluates the wellness of monitored individuals in real-time based on selected parameters. The monitor also issues mobile alerts when it detects data that deviates from a patient’s stated targets, personal baselines, or goals. The monitor is SaaS-based, standards-based, and interoperable with other applications.

A wristwatch-based monitor establishes a 24/7 electronic safety net by generating alerts for fall-related impacts or requests for help.  “Because the watch is less stigmatizing, frail patients are more likely to wear the monitor when they need help,” explains Jill DeGraff Thorpe, AFrame Digital Vice President for Strategic Initiatives. She also added “It also means that the monitor is able to gather activity data which helps care teams evaluate a person’s daytime and nighttime activity patterns.”
